Ishmael Parsai, Ph.D., Editor MPW Hyperbaric Oxygen Therapy HBOT ; has stood alongside radiation therapy for decades. The word "Hyper" means increased and "baric" relates to pressure. Hyperbaric oxygen therapy is a form of treatment that allows a person to breath 100-percent oxygen at greater than normal atmospheric pressures. In contrast to the earth's atmosphere which normally exerts 14.7 pounds per square inch of pressure at sea level known as one atmosphere absolute 1.0 ATA , the oxygen pressure in HBO treatment is increased to pressures two to three times normal atmospheric pressure. Patients breathe 100% oxygen under these pressures either by breathing the ambient gas oxygen ; in a monoplace chamber or by breathing 100% oxygen through a mask or oxygen hood in a large multiplace chamber where many patients are treated simultaneously. When we breathe air at normal sea level pressures, we are breathing a mix of gasses composed of approximately 80% nitrogen and 20% oxygen. While under HBOT, the increased pressure combined with an increase in oxygen to 100 percent, dissolves oxygen in to the blood and in all body tissues and fluids at up to times normal concentration--high enough to sustain life with no red blood cells at all. While some of the mechanisms of action of HBOT as applied to healing and reversal of symptoms of certain diseases are still under study, it is known that HBOT: Greatly increases oxygen concentration in all body tissues, even with reduced or blocked blood flow and; Stimulates the growth of new blood vessels in tissues with reduced circulation, improving permanent blood flow to areas with small vessel narrowing; Other effects of hyperbaric oxygen include the mechanical compression of gasses entrapped in blood vessels or body tissues. This effect is the primary mechanism operative in the treatment of air embolism and decompression sickness. HBOT greatly aids the treatment of infection by enhancing white blood cell action and improving effects of germ-killing antibiotics. Possible additional effects which have been postulated by some but as yet are unproven include the following: Rebound arterial dilation after HBOT may result in an increased blood vessel diameter greater than when therapy began, improving blood flow to compromised organs; HBOT may lead to an adaptive increase in superoxide dismutase SOD ; , one of the body's principal, internally produced antioxidants and free radical scavengers; Wherever blood flow and oxygen delivery to vital organs is reduced, function and healing can potentially be aided with HBOT. Hyperbaric oxygen is recommended by the Undersea and Hyperbaric Medical Society for 13 specific indications varying from decompression sickness Bends ; to diabetic foot ulcers. A common indication for hyperbaric oxygen is delayed radiation injuries including mandibular osteoradionecrosis.
HIV is a disease state in which the overwhelming number of people with the disease are black--it's not even a close call.As black doctors, we know to screen for hypertension, diabetes, cervical and prostate cancer because they happen disproportionately in our community. HIV has emerged as the leading cause of death among black women ages 25 to 44 and one of the top three causes of death for black men ages 25 to 54. We must encourage more patients to test for HIV--and get those who are ready, to start treatment.Throughout this special supplement to POZ, several other African-American HIV specialists and I share insights and ideas for HIV prevention, testing and treatment. As physicians, we've got lots on our plates. But a few key steps, taken individually, can have a vast impact in the fight against AIDS in the black community.
The reason to choose nPEP is the hope that using anti-HIV drugs within 72 hours of an exposure to HIV might block the establishment of HIV infection. The following is a list of reasons why you might choose not to use nPEP, despite having a high-risk exposure.
Children who are resistant to Kaletra will take HAART with or without an NNRTI. Everyone will also take Kaletra. After 2 weeks, they will have blood tests to check the amount of Kaletra in their blood. Dietary Cu deficiency is associated with impaired reproductive performance. Delayed or suppressed estrus has been observed in cows grazing Cu-deficient pastures 2 ; . Treatment with Mo reduced pulsatile release of LH and delayed onset of puberty, but treatment with Fe had no effect on LH, even though it depleted body Cu. Thus, the role of Cu in bovine neuroendocrine function remains unclear. Although peripheral blood T cells from these patients have been shown to secrete excessive amounts of interleukin IL ; -4 and IL-5 compared with that found in normal control subjects, the underlying immunopathogenesis is poorly understood 11 ; . Several investigations have demonstrated an association with atopy. For example, nearly half of the patients are atopic as defined by elevated levels of total IgE or food-specific IgE, and IgE-mediated mast cell degranulation has been demonstrated in patients with eosinophilic gastroenteritis 12, 13 ; . The exact prevalence of the eosinophil disorders of the GI tract remains unknown. Non-IgE-Mediated Food Allergy Non-IgE-mediated food sensitivities are becoming increasingly recognized. Non-IgE antibody mediated or associated food allergies represent a spectrum of clinical diseases attributed to adverse immune responses to foods for which IgE antibodies to the causal food protein cannot be demonstrated, at least not by routine tests. Several clinically well-defined disorders that develop in infants are perhaps the best demonstration of this type of food allergy 14, 15 ; . The onset of these reactions is slower than immediate IgE-mediated reactions, ranging from a few hours to more than a week after ingestion of the causative agent 16 ; . In some cases, even more prolonged and repeated exposure is required for the development of clinically apparent abnormalities. Inflammatory cell infiltration of the GI wall is seen in many of these conditions, but most diagnoses are currently made on the basis of clinical presentation and response to dietary exclusion 1, 5 ; . Identification of the causal foods for non-IgE-mediated disorders is a complicated undertaking. The symptoms are often subacute or chronic in nature and no simple tests are available to secure the diagnosis. Therefore, elimination diets and oral food challenges are the primary mode of diagnosis and ancillary tests are often needed such as intestinal biopsy ; . The atopy patch test and in vitro lymphocyte studies may hold promise for future improved diagnosis of these disorders 17, 18.
Infection acquired by healthy persons is usually asymptomatic or may lead to painless lymphadenopathy or a mononucleosis syndrome. Maternal infection is usually unrecognized. Disease in persons with depressed cellular immunity eg, persons with AIDS, transplant recipients, persons receiving immunosuppressants ; usually is due to reactivation of latent infection but can result from acute infection. Toxoplasmosis in these persons leads to lethal meningoencephalitis, focal lesions of the CNS, and less commonly, myocarditis or pneumonitis. The clinical picture may include headache, seizures, mental status changes, focal neurologic signs, and aseptic meningitis. Thirty to forty percent of AIDS patients with IgG antibodies to T. gondii indicating chronic latent infection ; develop active toxoplasmosis unless they take preventive medication. Congenital infection occurs when a previously uninfected mother develops infection during pregnancy. Infection prior to conception, demonstrated by specific IgG antibodies, in nearly all cases guarantees against infection of the fetus. However, transplacental transmission occurs from mothers whose prior infections reactivate when they receive immunosuppressant medications or develop AIDS. Congenital toxoplasmosis may result in abortion, stillbirth, mental retardation, and retinal damage. Recurrent toxoplasmic chorioretinitis in children and young adults is frequently the result of congenital infection that was asymptomatic at birth and hoodia.

J Psychiatr Res 2005 May; 39: 303310 Background: Limited efficacy has been shown with selective serotonin reuptake inhibitors SSRIs ; in the treatment of eating disorder psychopathology and comorbid symptoms of malnourished patients with anorexia nervosa. Recent data suggest that SSRIs may prevent relapse among weight-restored patients. Although some previous studies included patients in late adolescence, most subjects have been adults. This study sought to determine the effects of SSRI treatment in partially weightrestored children and adolescents with anorexia nervosa. Method: This retrospective study included 32 females with anorexia nervosa mean age 14.5 1.4 years ; . BMI, eating disorder psychopathology, depressive symptoms, and obsessivecompulsive symptoms were evaluated during inpatient treatment and at 3- and 6-month follow-up. At the 6-month follow-up, medication history during inpatient and outpatient treatment was reconstructed. 19 patients were treated with SSRIs; 13 subjects were unmedicated. Results: While patients in the SSRI group had BMI and obsessive-compulsive symptoms similar to those in the non-SSRI group, their levels of core eating disorder psychopathology and depressive symptoms were higher at the start of medication. Readmission rates were 36% in the SSRI group and 31% in the non-SSRI group p .72 ; . Repeated measures analysis of variance revealed no significant group with time interactions for BMI p .84 ; , core eating disorder symptoms p .79 ; , depressive symptoms p .75 ; , and obsessive-compulsive symptoms p .40 ; , indicating minimal or no effects of SSRIs on their course. Conclusion: These results challenge the efficacy of SSRI medication for eating disorder psychopathology as well as comorbid depressive and obsessive-compulsive symptoms in adolescent anorexia nervosa. Clinicians should be cautious in treating adolescent anorexia nervosa with SSRIs.

We have chosen to mask the assigned OC by capsule and this has been rarely done in OC studies since most OC research has been conducted by drug companies who can produce identical tablets when they wish. Encapsulation is the standard for other blinded studies and any change will affect all study medication tablets and be distributed across the entire sample. 14. Our study will not result in a branded or marketed pharmaceutical product. There is unlikely to be a profit reason for a company to pursue this work because both study OC formulations are off patent and even the continuous schedule is not likely to hold up as a patent given the long history of continuous OC use. Providers will have to prescribe the continuous use of the OC off-label, and this will necessitate 18 21-day packages for an entire year of daily use. It does not require a name brand to prescribe thyroid replacement and should not be necessary for menstrual cycle suppression. Perhaps someday these OC formulations can be dispensed in 31-day vials just as other daily use medications.
