Health counseling, legal assistance, and housing information for the homeless population of Knox County. UTCVM students volunteered with CAIT to provide veterinary services for the pets of those homeless citizens. CAIT also led a number of workshops and presentations at the state and local level. To educate citizens on issues facing companion animals in Tennessee, in fall 2005, CAIT held six public forums with guest speakers ranging from animal control officers from across the state to Judge Bill Swann from the 4th Circuit Court in Knoxville and Senator Tim Burchett. In April 2006, CAIT worked with the UT College of Law's Pro Bono Animal Law Project to sponsor a presentation by Dr. Bernard Rollin entitled "Animal Rights as a Mainstream Phenomenon: Animal Use and Emerging Social Ethics." Dr. Rollin is director of Bioethical Planning at Colorado State University and developed the first university course on veterinary ethics. In honor of Dog Bite Prevention Week, in May 2006, volunteers from CAIT and Human Animal Bond in Tennessee HABIT ; teamed with other volunteers from the college and community to hold dog bite prevention classes for children ages four to eight. Through education we hope to reduce the incidence of dog bites in children.

Woody * * AIDS Opiate Division, Center for Studies of Addiction, University of Pennsylvania, Philadelphia, PA and * INSERM U-379, Marseille, France Background: Follow-up studies of intravenous drug users IDUs ; are needed to evaluate public health interventions and vaccine or drug trials. Retention is a critical issue in such studies.Objective & methods: To evaluate a stepwise procedure directed at improving the 6-month retention, we analyzed the number of participants retained at each step and their characteristics, in a follow-up study of 263 seronegative IDUs at high risk of HIV-infection. Subjects were recruited from September 1997 to June 1998 in community settings in Philadelphia. Retention procedure: At -2 weeks, a reminder letter was sent; participant was phoned at -1 week and at -1 day or day 0. If participant failed to complete or reschedule visit, the following steps were taken until 6-month visit was completed: during week + 1: letters to participant at all contact addresses and daily calls; during week 12: outreach visit, letters, and at least 2 calls to participant and his her contacts; starting at week + 3: weekly calls and outreach visits, monthly letters, complemented by attempts to locate subject at shelters, food lines and prisons. Results: As of December 1998, 93% of the subjects completed a 6-month visit: 49% returned on time, 32% returned within 1 month of scheduled visit, and 12% beyond 1 month. Subjects lost to follow-up were more likely than those retained to have a high-school degree and to have moved frequently during past year. By contrast, subjects who returned with delay were more likely to have a low income and to live a distance from the study center. Overall, subjects did not differ regarding their drug habits. Conclusion: Although return diminishes at each step of increasing effort, sustained retention effort is required to conduct valid follow-up studies among IDUs at high risk of HIV infection. Socio-demographic characteristics are correlated with retention, and differ between participants late for scheduled visits and those lost to follow-up. Absence of correlation between retention and drug-related behavior might be due to our selection of subjects who were all at high risk of HIV infection. 323.

If you do not have a significantly increased cardiovascular risk profile, Compassionate Oncology feels that most prostate cancer patients should consider taking Celebrex. Contextualizing mental health: gendered experiences in a Mumbai slum Abstract Urban mental health programmes in developing countries remain in their infancy. To serve low-income communities, research needs to consider the impact of common life experience in slums, including poverty, bad living conditions, unemployment, and crowding. Our study in the Malavani slum of Mumbai examines afflictions of the city affecting the emotional well-being and mental health of women and men with respect to gender. This is a topic for which mental health studies have been lacking, and for which psychiatric assumptions based on middle-class clinical experience may be most tenuous. This study employs ethnographic methods to show how environmental and social contexts interact in shaping local experience with reference to common mental health problems. Focusing on the social and environmental context of the mental health of communities, rather than psychiatric disorders affecting individuals, findings are broadly applicable and sorely needed to guide the development of locally appropriate community mental health programmes. Identified afflictions affecting mental health include not only access to health care, but also sanitation, addictions, criminality, domestic violence, and the so-called bargirl culture. Flicek, NP Heel spurs and plantar fasciitis are often discussed together in the medical literature. It is difficult--and usually unnecessaryto distinguish between the two because the causes and treatment of each condition are the same.1 A heel spur is a bony projection on the sole or plantar region of the heel bone calcaneous ; . This condition may accompany or result from severe inflammation of the plantar fascia.2 The plantar fascia is a fibrous band of connective tissue on the sole of the foot, extending from the heel to the toes. Plantar fasciitis is the most common cause of inferior heel pain.2 The hallmark sign of plantar fasciitis is pain on the bottom of the heel that is most prominent with the first steps upon awakening.2 Plantar fasciitis is common in runners and overweight people who have recently increased their activity level. Plantar fasciitis is considered a chronic inflammatory syndrome rather than a posttraumatic disorder.2 Typical morning pain results from a cycle that begins with the foot resting in plantar flexion during the night, allowing the fascia to contract. The first steps of the day stretch the fascia, resulting in pain. This stretching results in micro tears at the calcaneal origin of the plantar fascia band. The pain is dull and similar to that of a toothache, and may be noted at the beginning of exercise or when weight bearing is resumed after resting. A frequent cause of heel spurs is an abnormal or excessive pronation of the foot during walking and athletic activities.3 In some cases, the sole of the foot flattens and becomes unstable. The portion of the plantar fascia attached to the calcaneous stretches and pulls away from the heel bone. Heel spurs are deposits of calcium that result from repetitive stress and inflammation. They are the body's response to injury and inflammation. Predisposing Factors Predisposing factors to plantar fasciitis and heel spurs are obesity, sudden weight gain, tight Achilles tendon, change in walking or running habits, change in running surface, and use of shoes with poor cushioning and arch support.3 To identify these factors, the history of every patient should include questions about occupations with prolonged weight bearing police officers, letter carriers, waiters and waitresses ; and any history of excessive pronation, pes planus or pes cavus. On examination, physical findings in a patient with heel spur syndrome or plantar fasciitis may include gait alteration such as supination of the foot to redistribute the load laterally or toe-walking to avoid pressure on the heel.2 The patient may also exhibit decreased dorsiflexion of the great toes and ankle. Mild edema may be present, and it may be possible to palpate a small granuloma along the medial fascial origin. No tenderness should be present with medial to lateral heel compression, and palpation alone should not produce tenderness. Foot motion that stretches the plantar fascia causes the pain. Diagnostic Studies If the patient does not have a history or finding of flat foot, highly-arched foot or excessively pronated foot, laboratory studies might be necessary to rule out arthritis or infection. Sedimentation rate, complete blood count and rheumatoid factor may be informative if bilateral symptoms exist or other joints are involved. It is essential to rule out conditions such as Baxter's neuritis, diabetes mellitus and peripheral vascular disease, which can cause similar pain with walking. Conservative Management Appropriate pharmacologic therapy for heel spurs and plantar fasciitis consists of nonsteroidal anti-inflammatory drugs NSAIDs ; for 2 to 4 weeks.3 Ibuprofen Advil, Motrin ; and naproxen Naprosyn, Aleve ; are recommended when there is no history of peptic ulcer disease, gastrointestinal bleeding or hypersensitivity. Steroid injection with triamcinolone Aristocort ; or dexamethasone Decadron ; may be appropriate in severe or resistant cases.3 Side effects of steroid injection include infection, fat pad atrophy or rupture of the plantar fascia. The leading conservative therapy is rest, especially if increased activity or injury triggered the pain.1 It is also beneficial to apply ice three times a day, especially after activity.1 Stretching the calf muscles before getting out of bed and after extended periods of sitting helps prevent reinjury. Taping, arch support, shoe orthotics and stretching exercises are helpful in conjunction with drug therapy and rest.1 Taping the bottom of the foot can keep it in neutral position. The purpose of taping and wearing orthotics is to cushion and support the heel and arch. Strips of tape placed along the bottom of the foot prevent the toes from splaying out and causing pain. Most orthotics provide passive dorsiflexion, which can also reduce pain. These are available without a prescription in office settings and through various Web sites. Generic heel cups and arch supports are available at most drug stores. Two helpful at-home exercises are massaging the fascia by rolling the foot over a cylinder-shaped item such as a rolling pin and bunching up a hand towel with the toes to strengthen the muscles that support the arch.

A review of Dr. Wolfe's medical report from October 31, 2006 indicates that he diagnosed claimant's condition as deQuervain's tendinitis and possible mild carpal tunnel syndrome of the left wrist. At present, the programs are DOS-based. Next month we will be releasing the Windows version to our states, and you will be seeing an overview of that system this afternoon. As you know, the implementation of a new ICD requires retraining of the staff that work with the data. Over the past year we have conducted a massive training effort. We have trained over 300 staff in our state vital statistics programs, and many staff in NCHS as well. Our courses include the traditional underlying and multiple cause of death coding courses and also a new course in ICD-10 coding concepts for statisticians. In addition to that, because the way our systems are maintained--they are maintained at a State level--we have developed a course for the managers of State PC systems. We received tremendous assistance from our Canadian partners. Ms. Patricia Wood, who is a nosologist in Canada, participated in the development of the training materials, and taught several of the underlying and multiple cause sessions.
